Levels of Care
Hospice care is tailored to each patient’s needs. At Creekway, we provide different levels of support to ensure comfort and peace of mind at every stage.
Routine Home Care
The most common level of hospice. Care is provided wherever the patient lives—whether at home, in a nursing facility, or assisted living. Our team makes regular visits to manage symptoms and provide comfort.
General Inpatient Care
For patients who need more intensive symptom management that cannot be handled at home. Care is provided in a hospital or inpatient facility until symptoms are stabilized.
Respite Care
Short-term care provided in a facility setting to give family caregivers a needed break. This allows loved ones to rest and recharge while knowing their family member is in good hands.
Crisis / Continuous Care
Around-the-clock nursing care in the home during times of medical crisis, ensuring comfort and stability when urgent support is needed most.
When to begin Hospice Care?
Hospice care is most helpful when started early. Patients and families often benefit from added comfort, guidance, and support throughout the journey—not just at the very end.
When Hospice may help:
